Only fill out this form if you are returning a pet adopted from GaDaH
OR
you have been invited to do so by a member of GaDaH.
Rehoming a pet is traumatic for the animal and the family, we try to make the transition as easy as possible. We do not judge, we just want to do what is best for the dog, which is why this is an in-depth form, with lots of questions.
By learning as much as we can about your pet, it will help to transition them into the rescue and we can also provide all information to potential adopters in order to find them the most suitable forever home in hopes that they do not have to once again go through this heartbreaking experience.
Although difficult to find, there are families that will adopt dogs that have issues as they have the time or experience to work with them. Please be totally honest and provide as much detail as possible, there is a text box at the end of the application to provide any additional information.
We do require full medical records to be emailed from your veterinarian and animals to be up to date on their rabies, vaccinations and annual tests.
Microchip registration is to be transferred to the rescue.
